Policies

Appointments
You can book on-line 24/hr a day and within 15 minutes of an available appointment times.  Always check to see if same day appointments are available.

Minors
All clients under the age of 18 will be required to have a parent or guardian present during the entire treatment as regulated by state law, 5.02 (8), 5.02 (9).

Late Policy
If the client is late, the appointment will be shortened and end according to the original start time of the appointment, and the client will be charged the full treatment fee.

Missed Appointments
Your treatment time is reserved specifically for you. If you need to cancel your appointment we ask that you give 24 hour notice.
Last minute cancellations or no-shows result in unnecessary gaps in the schedule. Clients who do not call within 24 hours will be charged for scheduled services*. 
Voice mail and e-mail is accessible 24 hours/day. You may call and leave a message at any time.

*If the appointment time is able to be filled with another client you will not be charged.

Payment
The client is responsible for payment of all fees. Payment is expected at the time services are rendered.
Receipts can be provided of payment for clients who need to submit proof of treatment and payment to their insurance carrier or their flexible medical spending account provider.
Cash and personal checks are preferred. VISA, MasterCard, American Express and the Discover Card are all accepted. Checks are made payable to: Nichole Cross. A $50 fee will be charged for any returned checks. Clients may pay for multiple treatments in advance if they wish to.

Gift Certificates
Gift certificates can be purchased for a specific length of treatment or for any dollar amount. The policy for no-show or cancellations of gift certificates is, if the appointment is canceled with less than 24 hours notice, the gift certificate will be considered payment for the missed treatment and will be considered null and void.

Client Rewards
Clients that refer others to this office will also be rewarded with a $10.00 off coupon when the new client uses your name as a reference. Word of mouth is the best marketing, and I am happy to reward clients that help this office grow!

Forms
All clients will be asked to fill out a medical history form. You may print this form and fill it out prior to your first treatment. By signing this form, you give permission to The Body Mechanist to send cards, newsletters or other clinic documents to the mailing address and email address listed on the medical history form. All contact information will be kept confidential.
